Solid Waste Management Facility Annual Reporting Forms

For all facilities regulated as Solid Waste Management Facilities, the 2021 Annual Report Forms are now available on-line on the New York State Department of Environmental Conservation’s (NYSDEC) website. The report forms are due by March 1, 2022. If you haven’t already started, now is the time to begin the process to collect information for the forms, to ensure that your reporting forms are complete by the due date. 


These Annual Report Forms are used by the State to establish where their resources are utilized, and plan which of their programs should be emphasized. Failure to submit these forms will result in violations that will cost your facility and reduce your profit. 

Forms are available for regulated facilities and are arranged under the following categories:
